I can't address the bolt torque issue, but reading some of the questions and
requests, including this one, I must plead with all to include thorough
design reviews, for instance at the 10%, 50-60% and 90% complete points.
When I do a 90%-stage design review, I follow a protocol very similar to
Process Hazards Analysis methodology (using, typically, a component by
component analysis of Hazop questions). If you are designing a refinery or
chemical plant you are asking for trouble (potentially a la Bhopal) if you
don't do thorough, methodical, and recorded reviews using experienced and
competent people.
