Hazop is one element of the PSM and normally focus on the hazard associated with the plant operations. In refinery or petrochem, design changes follow the other protocol - the Management of Change (MOC). But then again this is limited to the those changes that affects the process safety information. Design calc such as those of the bolt torque or thickness of the vessel and pipe to confirm design normally does not have to undergoes 30%-60%-100% designreview. I think that such review stages is best reserved for capital project.
