With regards to when a fig.8 vs Paddle-Blind (aka Spade & Spacer) are used, it is purely a question of size. On larger lines, it is impractical to have a fig 8 sticking out - they do take up a lot of lateral space - so a Spade & Spacer are used instead. The fuction of both (i.e. fig 8/S&S) are exactly the same.

A figure 8 is a two-lobed arrangement, pretty much like a blind flange and a spacer welded at their tangent. If the "blind flange" lobe is exposed (not between the flanges of the pipe), it indicates that the line is "open" and if the "spacer" lobe is exposed, it indicates that the line is blocked.

A paddle blind is basically a blind flange with a handle welded at it's perimeter.

It's been years since I've used a figure 8, but they were typically "built in", with a pivot arrangement, so you were pretty much required to have one lobe or the other in place. You had to pull most of the flange bolts to make a change, so it isn't something you did without discussion and planning, but it was a positive way of blanking a line and assuring it was done.
