it'll happen unless "standard/policy/procedure" are in place, training
conducted, audit perform, etc. We do have the procedure in place - from
lifting, to welding, specifying material, piping layout, perfomig FMEA,
conducting the Hazop, development of safety instruction, prcurement,
blinding, etc.
The development of engineering guides for equipment and system (covers fabrication, tsting and installation) will ensure that such incident dont happen and that the quality of work is "repeatable".
