Hi-Lo Piping FitUp

Situation:
Three week long turnaround is almost done. Remaining activity left is to boxup the piping and mech'l equipment. Operation is ready to startup the unit sytem by syten and all of a sudden QC Inspector says that the welds on two flanges ( 24"-150 ) for the cooling water service failed the x-ray. Flange is for the lug type butterfly. I beam support both side of the valve. Temp is ambient and pressure is low. Welds done on night shift using contractor. All are certs.

  Rework will take about two days and downtime is costing the company more than a million riyals per day.

Reviewing the film (no im not a level II ASNT certified inspector) and the fitup its pretty obvious that the "culprit" is the hi-lo fitup. Ive have recommended that we aceept the condition of the weld provided that a split ring with a tel-tale hole be welded as an insurance to enable Operation to startup the Unit early.
