More than likely xray was rejected because of shade line, or nonfusion because of Hi-Lo fit up. Forman should have a chat with xray or inspector to determine what it is. My guess is shade line, or nonfusion. Easy fix would be to unflange and back weld, may need to grind down bead before back welding to prevent excess.
