A study commissioned by PVRC (pressure vessel research council) indicated that most flange joint failures resulting in leaks are due to: Improper installation 26%
Flange damage 25%

Gasket 	 22%
Loose bolts 	 15%

Flange misalignment 12%

More interesting is the test performed by canadian based Tightness Testing and Research Lab (TTRL) regarding the performance of nine different type of gaskets (gore-tex ePTFE cord, spiral wound, gasket sheet, caf, etc). Guess what type made it to the top. Wrong. Its gore-tex by a wide margin (probably sponsored by gore-tex). but they're good (very good in fact)and the only limitation is the service temperature.
