The optimum thickness of insulation is based on heat loss/gain economics. Based on calculation using the temp difference between the pipe/tank content and environment, the difference in temperature etc, one is able to determine the optimum thickness of insulation.
Different type of insulation is used for hot and cold services. The cold service being concerned with moisture ingress into the insulation material, causing corrosion and other problems.

Bypass lines relieve the pressure across the valve. I suspect that its absence in globe valve applications may be because globe valves open against the back pressure, therefore requiring greater force while a gate valve will slide unaffected by the pressure difference across the valve. I await the real piping fundis to answer this one satisfactorily.
