As I understand, this is basically the technique used to stabilize the bell tower in Pisa, Italy (along with some additional underground supports). It must work!

But maybe the problem can be solved without the issues related to piling, earthwork and excavation.

Most civil contractors now have concrete pumpers. Drill a hole at an angle under the existing sleeper and slowly pump in the new mix under it. Be careful while doing this.

Depending on soil liquidity (and freeze climate) consider installing a 2" layer of Styrofoam under the sleeper.
