i used to think that there is only one root cause leading to failure. i was wrong. the root cause analysis (rca) has matured a lot since the last 2 decades - now its a very well structured tool to analyze failure data and events. in this case, some of the root ccauses we found leading to the nozzle failure are:

predictive mtce (t&i frequency)is less than adequate (lta). equipment design problem - design output lta. installation/fabrication. Inspection and matl acceptance req't lta. equipment misuse. operating procedures. design limits exceeded.

the material you are referring to just fall under one casual factor.

however The question remains unanswered. why the diameter as a basis for the equip nozl load allowable - more specifically to api 610, 617 and sm 23?

i found the answer in the early 80's (can't forget it. this is the time i was exposed in vibration analysis)when ive read a paper by Peter Simmons (Shell, UK)- A new approach to pump flange loading. talk more about it later. Received on Sat May 11 01:45:00 2002
