Bolt holes should straddle the horizontal or vertical centre lines or the plant north/ south centre lines when orientation is not given on the isometric.

I think there must be a rule like "alway orientate one bolt-hole in UP-direction (for horizontal lines) or N-direction (for vertical lines) in accordance with the ISOMETRIC"
With something like this there must be NO problems (if both Iso's are correctly done!!!)

flange
which is bloted with the first flange them an elbow which is going to
diferent angle with the first spool.
If the fitter do these spools as you said: orientate boltholes either side
of that" so how can these two spools match up on site since they going to
different angle so are boltholes?

ARE YOU BEING SERIOUS !??!!?! When the line is being fabricated as a spool, the fabricator doesnt know that the line is necessarily running NW (Very probably using spooling drawings). The bolts will still stradle the principle CL of the pipe. i.e. They will treat the line (running NW) as though it was running north and orientate the boltholes either side of that.
