We have experienced quite a few failures on the couplings . An audit was carried out. It was found that the couplings which were higly loaded gave way inspite of the deflections remaining within allowables. The couplings on vertical lines is a typical case. As a part of remedial measures the damaged couplings were replaced by special flanged joints (vendor proprietory).
