Regarding flow element installation in piping

Can anybody tell ...

About the upstream and downstream length requirement for the flow element ( specifically orifice flanges ) for the water service piping.. I know 10D for upstream and 5D for down stream piping length Is required .Is it applicable for all cases or any deviation can be accepted??

How this lengths are decided ? I will be thankful if someone explain in detail with some document references.
