Questions Regarding Flange Failure:
Flange material?
Is the flange in a corrosive environment in the plant?
Is there any potentially corrosive gas in the air line?
Did the flange fail through the thickness, or did the crack stop at around the midpoint of the flange ring?
Did they have any idea what the bolt-up loads were?
Has the system been recently hydro-tested?
What type of gasket was used?
Were there other cracks around the top, or are the cracks in the bottom the only ones?
