Looks like a lap joint (Van Stone) that has been drastically overtightened,
incorrectly fitted and/or the line has seen a slug or other shock. But it
doesnt look like a lap joint flange.
Lap joint flanges normally have a thickened neck to resist cracking at the
ID. Looks like a plate flange has been used instead.
If this is the case, then the flange may not have a radiussed inner shoulder
which would tend to cause cracking as the flange was forced on to the stub
end shoulder fillet.
Thats my guess - if its a lap joint.
