I don't see why this would be a maintenance problem (if I understand the question correctly - it's worded confusingly).

Operations and Maintenance always have ways to get around design mistakes and oversights - one example might be the expected travel of a gate valve handwheel when fully open.

Plant/piping designers will allow for the travel but often forget that there is often additional overhead space required for the removal of the valve innards through the bonnet (if that is possible).

A similar phenomenon happens when an inexperienced person wants to perform a hot tap as a solution. The machine (plus operator access) take up much more space than might be expected.
