The suggestions you have are considered good practice in all the area
where I've used bolts. I've always treated them as conventional wisdom.
The best reason I've heard for the 'two threads' extension is that the
first couple of threads are usually imperfect, so you get full
engagement only if you extend that far past the end of the nut.
