Tom,

   You are fortunately to have worked in the top end of industry. Things are well controlled there as they have to be. But things still go wrong.

   Once you venture into the murky waters of water and waste water, minreal processing, mining, building services you find a different story. Many a time I have tried to explain to people the need for P&IDs. Some just dont get it!

   Standards in these industries are contradictory.

   I am not talking of the high profile multimillion dollar projects where you might have a Fluor, Bechtel etc running the job. The smaller projects where second tier companies or authorities are the designers are the worst.

   I have had drawings issued with the same revision but have changed substantially, I have drawings that are from the same base issued with two different revisions at the same time by a controlling head office in the UK and the local office here.

   Let me tell you working in a controlled environment is easy. Like comparing city life to the outback.

My assumption is that people who earn their living by using PDS/PDMS or CII (this is piping design isnt it) or just involved in piping engineering are also following certain policy/procedure governing their work like how to initiate a project or issue a design packages.

In addition, these very same people is familiar with the working of not only the applicable Code that applies or the use/selection of proper material but also the SHE requirement in addition to their inhouse  management system - most especially if the project deals with handling hazardous chemicals.

Is my assupmtion right? Piping drawings are normally a controlled documents.
