Thats OK with me Geoff and by the way merry christmas. When i say
putting in place a managent system - i meant a system that works and
subject to an independent audit/verification. A workable system is a
system that is supported by management and that means providing the
resources that it needed. It also means that the people believe in
it, theyre properly trained and are aware of the consequences when
they ignore it.
Investigation after investigation of major plant catastrophies
reveal that in a lot of cases the root cause is the lack of a
management system (that works).
Few more sleep and its 2006 - and hope that you guys have a
prosperous new year. and if you have some extra change, please
donate to the victims of tsunami - most are still suffering.

companies named has testing of their relief valves running six
months behind schedule and the management have adopted a"higher risk
profile" to accomodate. They are amking too much money with the
price of petrol at the moment and wont allow a shutdown to proceed.
Another is operating pressure vessels above their rated pressure and
pops reflief valves when pumps stop at the other end of the pipeline.

companies have all the paperwork but unless the troops are given the
resources to follow the procedures its a waste of time.
