My thinking is bit different,
We should consider operability also. Most of the time small vessels have operating valves attached to nozzles, we may make these valves difficult to operate. Nozzle should be easy to approach from walkway. Instrument nozzles (Pressure Indicator, Level Gauges) should be placed so that operator can read it easily from walkway.

YES most of the times & NO Some times.
When a minor equipment/pump located close to major equipment, and if the
fluid temperature is relatively high, "Nozzle will be located away (little
bit), i.e. not exactly towards the equipment"
Reason: By doing so, piping layout generally comprises of 2 or more elbows
which makes the layout flexible.

Your first ans, is Yes, there is one rule which follows every nozzle orientation that what ever piping is done must be travel minimum distance and with supporting facility so nozzle must be kept in htat direction. generally the nozzle piping attachment nozzle kept in piperack direction. then manhole wich is kept towrds walkway direction.
generally octagonal foundation is placed on the equipment where there is not more no of nozzle at top.
and Piba is the best sofware which i suggest best to calculating mto.
