As per the information, it was a hot tap of 36" gas line. but not sure whether it was at the time of welding or tapping operation.
There are lots of technicalities to be followed during the welding of fittings and during tapping operation times. Thats the distinction of the specialists in this field.
As I quoted in the earlier discussions, we have 6 nos. of hot tappings in our ongoing HP and LP gas pipelines, 8",36",42",50"..... Out of that 42" completed successfully ,which was performed 5 days after that saudi blast.

Was it a hot tap, that failed?
Hot tap is a common thing in Aramco. One of the things that is done is the 90 % yield pressure test of the fitting assembly.
