Flange bolt-holes orientation

Unless otherwise noted, piping spool fabricator will assemble flange so that the bolting holes straddle the principal center line of piping or equipment. so what if I have:

  1. Piping runs from SE to NW then an Elbow tuning straight down, on the end of Elbow there is a flange. What orientation the bolting holes will be?
  2. piping runs from SE to NW. In the middle of this piping run, there is a Olet type branch, after the Olet there is a flange. So what orientation the bolting hole will be?
