First Name talking about accidents that comes to my mind in refining was TOSCO.... They worked wonders out in CA.....

Rumor has it that those poor guys @ the BP refinery were in a required safety meeting at the time of their death... another bit of mindless bureaucracy gone wild... lets talk about something but if it costs anybody any money well thats another thing!
