Have any of you guys heard/read the incident that happen in Germany way back in January 2005?

It was about the propylene leakage at the Olefin plant. Initially they suspect that the leak is at the flange joint. When they strip the insulation, whoa, its about a 4 inch crack at the flange neck. Ive heard that it was cause by vibration. Not sure if its BP plant.

By the way, anyone has the latest news on what really happen at the BP refinery in Texas. Previous bulletin says that before the fire, maintenance has repaired a 3/4 inch bleeder valve on a furnace line. They suspect that the fire initiated at the same location few hours later.
