Im pretty sure BP has also a published standard guide. So does the Piper Alpha, Chevron and others. And yet the worst disaster at the plant happen in these places. We have dozens of engineering guide and a few dozen weld specs and still getting few citations related to plant jobs from an independent safety auditors. Cant be the standard requirement.
