I hate to contradict you at this time of the year. One of the companies named has testing of their relief valves running six months behind schedule and the management have adopted a"higher risk profile" to accomodate. They are amking too much money with the price of petrol at the moment and wont allow a shutdown to proceed. Another is operating pressure vessels above their rated pressure and pops reflief valves when pumps stop at the other end of the pipeline.
The fact that nothing has happened is pure luck. These big companies have all the paperwork but unless the troops are given the resources to follow the procedures its a waste of time.
