While we can supply some "wye" outlets factory-welded on pipes, I have not before seen wye pressure taps (nor readily available sleeves made to accomplish same) --- while what you are looking for may well be out there, and I will wait to see, I just thought I'd ask if there is there any possibility you could make a conventional lateral hot tap with conventional equipment/procedure a little to the side of your intersection point, then bolt a 45 bend onto the branch tapping valve?
